If you are admitted to a hospital that has an agreement with SMA Neutra, you must contact your insurance company before your admission.

A certificate of coverage will be sent to the hospital and a copy will be sent to you.

A claim form will also be sent to you; it must be completed and returned.

⚠ In an emergency, the hospitalization declaration can be used provided that it is completed before you leave the hospital.

⚠ If the hospital where you are admitted is not one of our contracted institutions, please complete the claim form.
